1It depends upon whether the drug is metabolized by the kidneys or in the liver. If either system has decreased function, that means that the drug will be cleared more slowly than usual, the drug level in the body will stay higher, longer...So the dosage of the drug is either decreased or the drug is given less frequently. In hospital, the kidney/liver function can be checked by daily bloodwork, and the drug's level in the blood may be checked also. Sorry, no specifics available in my head, I'm not a MD or PharmPhD....
